I had the same problem. After trying several brands, the only one which is working perfectly is Maxell (yellow discs). I found them is the US (Seattle), have not seen them in France where I am leaving. Hope it can help !
Not sure which code you mean. If you are talking about the region code, you need to use the service remote, which is available only to dealers. However, you can do a search and find that there is a company in UK that will rent you a service remote for this purpose. Be prepared to pay up to $50 for this service.

Try using a different cable for starters, regular rca, another optical or coaxial.
If the optical works, then its a cable problem.
If the coaxial works(and another optical doesnt), then its a cable/jack problem.
If the RCA works(and the rest don't), then its a problem with the digital output.
Try blowing into the holes, and if nothing works and you can't find any other advice, contact Pioneer.
