You need to clean the lens or ajust the power going to it, to clean it all oyu have it do is get one of those "laser cleaners" from any tech store. if that doesnt work then it's time to viod a waranty(if its still under waranty get an exchange!) depending on what type of ps2 you have(original/slimeline and the the firmware) you will have 2 screws to ajust the laser's power. Clockwise is more, counter-clockwise is less. the screws should be located under you drive(this could blow your laser so if you feel uncomfortable, take the top of the drive off and ajust a white cog wheel that controls the laser's angle. make a mark of its original location and turn it clockwise untill you can read the disks again. if you turn it too far it will fall back down)

Sounds like the Xbox 360's disk drive is starting to fail. Knocking on the top of the Xbox (when its on its side) while loading a game can sometimes get it to work, but its better to have your Xbox repaired or replace.
1. download and install a psn game or game demo
2. put in the disc of the game or video you want to play or watch
3. load the psn game or demo as if you were going to play it
4. quit the demo as soon as you get to the start menu of it
5. go and play or watch the game or video you wanted to, sucka, cause it works now!!!

sounds like the reader lens has burnt out on it .is your xbox stood upright ? seem to hear the lenses burn out alot faster if the consol is upright as opposed to being led down, you can buy the parts from ebay or other computer shops , or send it back to microsoft to repair , you are able to lay downloaded games as they play off of the hard drive memory , where as the disc games obviously read off of the disc when the laser gets tired they work intermitantly then eventually stop all together.

Unless this is a game you "installed" from the online store, then all other PS3 games require the disc in to play.
With the disc in the system, go to the Game heading in the menu. You should see a disc icon with the name of your game...now click it. If you do not see the image, you will probably see text that says "disc read error". In this case you need to either clean the disc, or replace the disc with a non-scratched or non-defective one.
If your PS3 does this with multiple non-defective discs, then your blu-ray drive may be defective.
I'm also having this problem as well, it has to do with the OS and it is corrupted. It seems like the "Game mode" has been corrupted preventing us from playing games on the hard drive or reading games.
