Here is the link for the user manuals from zenith - they dont have one for your exact model, but if you type in C13A03D you will get a manual - page 34 details all the v-chip settings and should be nearly identical to your model at least;

V-chip Block TV- 14 - this is a rating for movies and TV programs. Basically means PG-13 like the old movie theater rating.
lsv- means, language, ***, violence.
You TV can be set to block these programs. In you menu look for a catagory called "locks" or "ratings"
there you will find the options of what can be veiwed and what will be blocked. if you everything to be available, just disable all settings. if you want to choose different things to be allowed, just click on the icons and they become blocked.
remote code don't go by model, you get a list of codes and you just try them one at a time until one works, here are the most up to date remote codes for a Zenith TV.here are the three digit codes
101, 142, 149, 177, 241, 345. remember all universal remotes have code search, so if none of these seem to work do the code search function, it is outlined in the user manual of your remote.
How do you KNOW the controller is Good??
There is a little Infra-red receiver behine that little glass window on the front of your TV. This has been known to go bad (assuming all those things are good that you said are good!)

If you have a remote that can access menu and your remote has number buttons and an enter key, you can follow this instructions to go to installer menu. Note that the installer menu is slightly different for each model, eg. some options are not available.
I was able to go to installer menu using two universal remotes. The remotes that I used were Dish Network 3.0IR and RCA RCU300WBL. Both remotes programmed for Zenith H2531DT TV. The reason why I had to use two remotes is that Dish Network remote was only able to access menu. It did not have ENTER key but RCA remote had ENTER key but I wasn't able to go to menu with that.
To go to installer menu (different from regular menu):
- Hold MENU for 8 seconds. - Press 9, 8, 7, 6 and ENTER in the sequence. You should be in the installer menu now. There should be a line that looks something like this: 03 VERT POS 13
Use SELECT button to scroll through each option, and ADJ button to adjust the values for each option. For example, to enable the front panel buttons on your tv, set KEY DEFEAT to 0. Other options that you might want to check:
CHAN LOCK which locks your TV to a single channel and wont let you change (set 0 to turn off) START CH/VOL will set your tv to a specified channel when you turn on tv. (set 255 to disable)
You will have to look in the menu for some of the options because they are different for different TV series. A good place to find manual is on www.zenith.com or http://safemanuals.com/
I work in an old hotel and it took me few hours to figure out how to disable start channel lock. It drove me nuts. I hope someone finds this useful.
