YOU HAVE THE WRONG SETRVICE CENTRE NUMBER IN YOU NEED TO RING YOU PROVIDER AND GET THEM TO SEND SETTINGS OR THERE GUIDE YOU THRO IT OR IF YOU HAVE THE SERVICE CCENTRE NUMBER GO INTO STTINGS THEN TXT MESSAGES THEN CENTRE NUMBER DELETE AND PUT NEW ONE IN AND SAVE THEN IT SHOULD WORK

The batery may not be holding a charge anymore, try removing the battery and clean the contact's, if that do'es not work the battery may need to be replaced.
I think there is some problem @ your battery. Change the battery & check whether the screen works or not if it ask for unlock code you can get the code from motorola code
While not a fix for this problem, what I have found to work nearly every time is to set something slightly heavy on the charger prong where it enters the phone. Turning it over and resting the prong on a book also allows it to make the necessary contact. It has lasted me for two years; one year charging just like that.
