All other sounds are ok on the phone - but when I receive a call I can barely hear them. Sound up to max level - ringer works, music works just cant hear caller.

Could be that it's just a software hickup: press menu and on/off button togetherfor 5 sec. If it's the same after the restart you need a new earspeaker. You can also use a headset, if it works with the headset, you definitely need a new earspeaker.There is a step by step guide too that explains a little bit at eachsteps what you have to do : iPhone speaker repair. If you want to change volume of the caller's voice, then try adjusting the volume control WHILE you are actually talking on the phone during a call.
If you try to adjust the volume when there is no call in progress, then the volume controls will only change the ringer volume.

separately on each handset or the base.
With the phone in standby, press or on the handset or VOLUME or
VOLUME on the base. The handset or base sounds the ringer at that level.
Use on the handset or VOLUME on the base to make the ringer volume
louder. Use on the handset or VOLUME on the base to make it softer or
turn it completely off.
When you hear the ringer volume level you want to use, stop pressing keys.
The handset or base will use the new ringer volume starting with the next incoming call.
