The following is the procedure to re-activate the radio if the display
shows "SAFE". It is important that the procedure be followed in the
correct order.
1. Turn on radio. The radio display shows "SAFE".
2. Press the FM2 and RDS buttons simultaneously and hold until "1000"
shows on the display. Release the buttons. Do NOT press these buttons
again, otherwise the radio will consider "1000" to be the code.
3. Use Preset buttons 1 ... 4 to enter the four-digit code number
from your radio card. Preset button 1 corresponds to the first digit,
Preset button 2 corresponds to the second digit, etc. Each press of the
button increments the number by 1.
4. Once the code shown on the display is correct, press the FM2 and
RDS buttons simultaneously and hold until the word "SAFE" appears in the
display, then release the buttons.
5. The radio should unlock and be usable shortly afterwards.
6. If an incorrect code is entered, the word "SAFE" appears, first
flashing and then continuously. You can repeat the above steps to unlock
the radio once more, the number of attempts will be shown in the
display. If an incorrect code is entered again, you radio will be locked
for about one hour. After one hour - the radio must stay on and the key
must remain in the ignition lock - if the display goes out, the you can
repeat the above procedure to unlock again. The cycle - two attempts,
one hour lock-up - still applies.
7. If you lose your radio code, contact your Audi dealer.
Note: Some European market Concert radios have a TP button instead of
the FM2 button, so the description above with those units would require
pressing the TP and the RDS buttons simultaneously.

Did you disconnect the battery or run it down? This message means you must enter the correct code. If you do not have the code contact your Audi dealer. I'm not sure which model you have but these are the instructions for Symphony 1. I'm not sure if this also applies to Symphony 2 (which has a built in single slot 6 CD changer)
Switch on - display shows SAFE as you indicate. Press and hold TP and RDS buttons at same time until 1000 appears. Release buttons (dont touch again or 1000 will be entered as the code). Using station select buttons 1 to 4 enter code. Use button 1 to select first digit of security code, button 2 for second digit etc (buttons 5 and 6 cannot be used). So, say the code is 5238, you'd press '1' five times, '2' twice, '3' three times and button '4' eight times. Once code is entered press TP and RDS buttons together and holde until SAFE appears Release buttons and a radio frequency will appear in the display.
If you accidently enter the wrong code the word SAFE will start flashing. You get ONE more chance to go through this procedure and enter the correct code. There is a more complicated procedure to go through after this with one hour time delays between attempts.Curtorsey Audi Owner's Club:

You must get the 4 digit security code and enter it into the radio by pressing the AM and FM2 buttons at the same time. Then enter code via the preset buttons. Press button #1 until you get the first digit of the code then press button #2 until you get the 2nd digit and so on for the 3rd and 4th digits. when code is on the display press the AM and FM2 buttons again and radio should come back on. if it reads safe you have the wrong code #. You can get the code from a dealer for a small fee. They need to remove the radio to get a serial # on the radio chassis.
I have a Saab 900 -97 with a similar problem. I just solved it yesterday on my car. Hold the Band button in for like 10-15 sec when you see "Code In", It clears the error log in the radio. If it doen´t work, enter a wrong code so the code hangs on the display and then press the band button to clear the error log. It worked on my car, good luck!
