It could be due to the printer driver not installed properly. Try to uninstall your existing Lexmark printer driver first before installing the new driver. To do that, go to Start menu > Programs > Lexmark X5400 Series Printer > Tools > Uninstall. After the uninstall process, restart your pc when prompted. To re-install the printer driver, please visit the Lexmark website to download and install the
Lexmark X5495 Printer Driver on your computer. Select your operating system
> click on "FILTER" > click on the driver under Recommended
Downloads > download and execute the file. Simply follow the instructions on
your computer screen to complete the installation. Then try to print a document to verify that the printer driver is properly installed.
Uninstall your existing printer software from your computer. From your computer, go to Start menu > Programs > Lexmark X5400 Series folder > Tools > Uninstall.
After the uninstall process, reboot pc when prompted.
When pc turns back on, check if your system is running on 32-bit or 64-bit. From your computer, go to Start menu > right-click on Computer > click on Properties. Take note of the System Type.

Printer manufacturer doesnt recommend use of refill due to this kind of problems. Most of the printer nowadays the head is actualy in the ink itself so it might be possible that the cartridge is already damage/touched during refilling it. My suggestion is to purchase new cartridge or better yet use a continuous ink cartridges, rather than refilling the cartridges.
