Have you selected the speaker A or B option. Its close to the power on/off switch. When you turn on the power of the PMA, you should hear a click from the amplifier indicating the protection circuit has given a go signal and all is OK in the output stage.

Might just be a stuck key or switch. Take a dry cloth and gently rub and press down on the keys like you are wiping it off. Then, if there are any switches on the phone, click them back and forth a few times. Then, take a spray can of air or a small air compressor (100PSI or less), and blow around the edges of the keys/switches. Good luck.
I have one of these too and it cannot be used with media player 11 as there are no updated drivers. Use media player 10 and you should have no problems.
