If I draw curves, swirls or straight lines, the brushstroke won't appear until I've lifted the pen, and there's always a line connecting the end of the brushstroke to the beginning. I've already tried uninstalling and reinstalling the driver, but it only temporarily solves the problem.

I had the same problem way back with my wacom tablet. Mine was due to mouseware drivers. Soon as I got rid of them it was fine. So if you have mouseware drivers it may be interfering with the wintab drivers

its the nib that is causing the problem - did you replace it or take it out recently? try taking it out and making sure that when u replace it or put it back in that nothing (dirt, fluff, whatever) is obstructing the passage way that the nib fits into.
hope this helps

I also solved the problem by installing the driver from the website. The trick was to uninstall the old driver first.

Go to your Brushes.
Click on the Brush Tip Shape tab.
Make sure the box is ticked that says 'Spacing' and set it as low as you can (1%) for the smoothest effect.

The contact switch in the pen is likely stuck. Test it with an ohmmeter.
It may have moisture or contamination or broken contact mechanism.
If the pen is anywhere near the tablet, the digitizing of position continues and if switch thinks the pen is down due to failed switch, the drawing will continue.
