Hi folks. I am the originator and have the solution to the problem. I feel a little stupid because I overlooked using a plastic insert/guide which is placed on the scanner, fitting on notches. It lets the scanner know to use the transparency device and provides a notch for film holder placement. It has been so long time since I used film ever since digital photography took over, I overlooked using this plastic insert. With it placed, I can scan film fine.
Thanks

Most likely it is a spec of dust on the scanning sensor. I recommend cleaning with a soft cloth and isopropyl alcohol. More info here - http://www.willshak.com/scanner/
