Shoulds like a issue with your ECM, check the ground wires make sure they are all clean, you may need to change the ECM out.

Did you put a new cap on the radiator? Small as it seems, if it does not provide an adequate seal on the radiator, than the system does not get pressurized, thus keeping the boiling point around 212 deg. your engine gets way hotter than than normally. When sealed right, the pressurized system raises the boiling point to close to 300. thats how is works. If thats not it,(assuming it is the last thing replaced) than you might have either a cracked head/ head gasket/ valve guides, cooling ports, basic large dollar items. Lets hope for your sake the problem is still in the cooling system, before you look at the motor. If you take out the sparkplugs, you'll be able to tell real quick, rusty, burnt whitish-orange looking condition. Good luck

does both the hoses get hot bottom and top? this will tell you if theres flow through the radiator. If you have a voltage tester test the wires on the fan connecter check there is current going to the fan if so replace the fan its a common problem as previously stated.
if there is no power to the fan when the vehicle should be turning it on i.e. its hot enough check the fan relay under the hood.

How often has the coolant been changed? My only guess is that it hasn't and the hoses and the heater core are plugged up with goo and a flush is needed

If the "Heater In Hose" gets hot, and the "Heater Out Hose" does not get hot, than either the Control Valve is sticking closed, or your heater core is plugged up with ****!
Have you used "Barrs Leak" or a simular product for a radiator leak? That will sometimes clog the heater core.
I have a 1993 Grand Cherokee, which "Normally" throws very hot air out of the heater, but lately sometimes it only throws warm air, till I make my first stop, than it gets hot.....all because the heat control valve is hanging up.
Low water in the radiator will also cause a heater to not work.....if the water level is too low to circulate to the heater.

where the coolant is going and why the gauges go nuts are probably not the same problem. sounds like your gauges have a problem with the ground side connection to power. you could look at the negative battery post and see if it is really gunked up, might as well check the positive side while you are looking. if the battery posts get oxidized pretty much everything gets a not so great connection but gauges are one of few places you notice it. the car only runs off the battery at startup or if momentarily car is using more amperage than alternator can provide, but current runs through battery from alternator so bad connection there will cause whole system to function less than optimum. in the cooling system one component you don't mention replacing is the heater core. you say the coolant is not on the ground, so where can it be? pull your oil dipstick and see if your oil looks like oil. if it looks like vaseline, kinda white and creamy, you just figured out where your coolant went and you have a blown head gasket. hope that's not it, it's sort of a hassle. there aren't too many other places for the coolant to go. it either runs on the ground, boils off against the hot block, like if it comes out near an exhaust manifold (and that makes quite an odor) or gets into the oil through blown head gasket. it can make it into the passenger compartment through a leaky heater core and if that happens you will smell a sort of sweet smell in the car and after while get a green cast to condensation inside windows. hope that helps.
