PUK codes are only available from your SIM provider.
You might want to check the original SIM documentation as the PUK code is sometimes included. The SIM provider will only supply a PUK code to the registered SIM user and will ask security questions to verify that you are the registered user. If you have a monthly contract then your SIM is automatically registered, but if you have a prepay credit SIM it's only registered if you have called the SIM provider to do so before you lost access to it.
The company will not provide a PUK code under any circumstances for unregistered SIM cards, so if that applies to yours then you lose all data stored on it (including unused credit) and cannot use that phone number any more. In that instance your only fix is to buy a new SIM card (so you'll get a new phone number as well), if it's the same brand as the old SIM then it will work in your phone, if it's a different brand then you may need to pay another company to unlock your phone to other networks.
