I my mind the actual lock bolt was assembled wrong. You need to gently twist the length adjusting unit /bolt directly out of its housing and then gentle separate the two pieces though fully separating them and then "flip" the actual little toggling metal piece that has the "U shaped and half circle looking hub that the lock shaft slides into, around from it currently position ... ie. reversing its throw. You may need to also adjust the spring metal piece underneath it as it run in a slot as well.
This worked for me ... when t-turn key is horizontal the dead bolt is extended ie. in locked position.

Does the motor run when activated? If so, your friction wheel "O"ring is worn out. This is a mechanical issue and should be covered by the Lifetime Warranty.
Call them. You only need to send the battery compartment side in. (inside of door)

I have installed your Powerbolt, but when I push the Lock Button, it unlocks? What should I do?
First remove the cover on the inside of the Powerbolt unit. On the right hand side under the batteries, you will notice that is says "Door Direction Jumper". If the cap is mounted on Prongs 1 & 2, carefully remove it and move it over so that it mounts Prongs 2 & 3. A is right handed, B is left handed.

Hi... You have too be sure that the deadbolt is free when it go inside the deadbolt hole... however this system is very low power and the (over torque clutch) will sleep if the deadbolt it's not completely free.
Also if you try to open the door before you unlock it, you may block the the deadbolt inside the deadbolt hole. Take care too unlock the door before open-it...

Good lock or good unlock

Very common for the keypad to stop working.
check that the keypad is not defective. You can disconnect the batteries and then unplug the keypad. Now put the batteries in without the keypad connected. If the beeping stopped, your keypad may need replacing.
You can get a replacement at the store where you bought it for free or call the Weiser support line and they will mail you one for free.

You should replace your batteries as the motor that moved the deadbolt does not have enough power to move it with weak batteries. If that fails make sure the deadbolt moves freely when the lock is removed as the mis-aligned mounting will cause excessive force be exerted to move the deadbolt. If this is an electronic lock, you may need to replace the batteries or check that the power supply is not frazzed if it is connected to the mains.

If it's mechanical, a couple of good squirts of lubricant might get it moving.

Other than that, you might need to call a locksmith.

Recut from another key.

If no keys, a locksmith will put in a new cylinder in with a set of three matching keys. My whole hotel in the UK was fitted with Weiser locks.

I'd try a new battery to "rule out" that as a problem, sometimes the contacts need a "turn" of the battery to make good contact.
