I think the problem is virus, you will need to flash the phone with a special software that will help reset the phone. Take the phone to any local phone repairer within your vicinity to help use the special software to flash your phone, once the phone is flashed, the problem should be fix.

The PUK code is on the Sim and not on the phone, even if you put the sim in another phone it will still ask for it, you only have 10 attempts on most sims to enter the code or the sim will be blocked...
To get your PUK code all you have to do is call your network and provide them with your phone number, they will give you the code unique to your sim :) Some networks also have the option on their website, however with some you have to be registered online to get the PUK online, registration involves sending a message to the phone, which in this case you cant read, lol.... Best just to phone them :)
