The LED Panel is dead.
Alesis has a parts company that they can refer you to for the proper LED panel. Price is high, however.
Contact Alesis 401-568-3131 or NuMark for the Alesis contact info. I was able to find QS6 and QS8 parts through this resource.

Try a factory reset:
Performing a 'Reset' on your Sr16 will will 'clear' it's memory and re-initialize all of it's parameters to the original factory settings. This implies that:
All of your User Patterns will be erased.
All Songs will be erased.
All drum assignments you have made will be erased.
None of the above actions should be an issue to you as:
It is assumed that at some point you have Download And Save A Copy Of The Sr16's Current State.
It is assumed that at some point you have Export any 'User' Patterns you may have programed into your Sr16 and imported them into your _rMidi installation.
It is assumed that at some point you have Export any 'User' Kits you may have programed into your Sr16 and imported them into your _rMidi installation.
Perform The Reset
To perform a 'Reset' on your Sr16:
Turn the pwer off and wait a few seconds.
Press and hold the PLAY and ERASE buttons while turning in the power.
Continue holding down these buttons for at least three seconds after turning on the power.
If this doesn't fix it, check power adapter. If that is OK then service is required

Come from the main outputs, probably labeled L/mono and R and connect those to your mixer for your audio system on either stereo mix input OR two two inputs that are panned opposite.. DO NOT connect this to a consumer type stereo system as you may blow the speakers or amp. Consumer stuff just isn't able to take output from music equipment such as this UNLESS the volume is kept very low.
Sound output is heavily distorted.
Check following things:
1. The master volume control on the SR16 is at least half way up. Turn down the amp as needed. Running very low output of the SR16 and high amp gain is sure to get distortion.
2. Check power adapter is the original and is producing correct voltage.
3. Check that audio cable is high quality instrument cable and not consumer or speaker cable.
I plugged the USB to MIDI lead input nd output the
You should NOT be able to damage the unit by reversing in and out on MIDI connections.
MIDI connections are fairly safe against damage unless you drive a ground loop into the output ones.
ALWAYS operate any connected equipment from the SAME power source receptacle.
Fopr instance, do NOT have a musical device powered froma wall receptacle across the room and connected to a computer on the other side as an example.
