Most games have the serial number for the online pass on the back of the instruction manual or on a seperate card. If you purchased the game used, you may be out of luck unless you purchase an online activation via PSN.

That means that you were on a private server and either that person kicked you for too much lag (slow connection between your connection and his), or most often that is what happens automaticly when a server admin sets ping time (lag) limits to bump players who weren't getting a clean connection to the server, and the system says you were kicked by the server admin who set the limit.
Try a different server with players located closer to yourself.
Make sure there are not too many people using heavy downloads or streaming applications at your home or on the same network.

setup ps3 account to log in automatically then ...
if you connect via a network cable :unplug the network cable before starting game,then once in plug it back in.if you connect by wireless : turn off hub, start game turn hub back on.
Wait to see your account login in top right corner then start playing normally.
Activision still need to fix this so you dont have to faf about.

Most of the time when this happens it means there is problems with the data saved already to your hard drive from the game. The bad thing is the way to fix it is to delete all data saved for that game as well all updates for it and reboot your ps3 then put the disk back in and start over....

You have to disconnect from the internet to play the game. If you use a wired connection - disconnect the ethernet cord - if you use a wireless connection then disable the connection in the settings menu.
