SD card cannot be read by windows?
Right click Start button to open Device Manager, find the SD card reader, open and choose Driver tab, try Roll back. If not available try Uninstall, restart PC to reinstall Driver.
Next right click Start to open Disk Management, find SD card on map at bottom, right click to assign a drive letter.

Has worked before and stopped working or never worked before?
If not worked before ensure your output ie HDMI has the correct converter cable or adapter to connect to monitor AVI or VGA port ie blue or white socket. also check the cable is ok try changing cables, or using a different port. test the monitor with a pc to ensure the monitor port is working test the ps4 with another device to ensure the output from the ps4 is also working.
