Upper Spray Arm Fell Off
If you are referring to the arm on the top of the dishwaser (not the one under the top basked) it snaps in. What probably happened is that it fell off and the plastic nut flipped around. The gray plasting thing on the top (that looks like a flower) will unscrew and you can turn the nut around and snap it back on and then re-attach the entire piece.

A paper brochure with schematics, parts list and service diagnostics was inserted behind the control panel at the factory in mine. To get it out, shut off power at the breaker box; lay the door down horizontally; unscrew the two screws (now facing up) in the door that hold the ends of the control panel to the interior edge of the door. (You may also have to unscrew any along the top edge.) The control panel will drop away from the exterior of the door. The paperwork will be under a clip near the controls.

Upper rack dishes not coming clean.
You could try running it with the upper arm removed, just so you could be sure that the pressure is not getting to that point - unless you are absolutely sure the upper arm holes/channels are all clear then descale it overnight with some vinegar - just might clear something.
if there is obviously only a little flow of water then perhaps you could try getting vinegar into the supply pipe using a basting bulb/syringe or similar - again leave overnight and you may get lucky.
The next thing would be to take the machine apart so you could follow this hose through to the pump - undo it from the pump and blow through it or see if you can get a piece of wire in it from the pump end. try vinegar again. check the pump outlet for blockage.
Hope this helps :)
