I insert the AC adaptor into the DC IN jack of the
try gently cleaning the 'eye' on the disc player very gently with a qtip and rubbing alcohol
this should get the player to start reading discs again
if not make sure you are using disc formats that are compatible with the player you are using (eg. DVD+R, DVD-R, DVD+RW, etc)

When trying to play dvd,
This might be a lot of things. THE first thing that you need to do is try several disks. If you get the same error on all of them EVEN CD's then you have to check and see if the head is moving and if the laser is clean. You should be able to do this when you open the unit. THe head and the laser should be visible if the lid for loading pops up. The other kind that you have to insert the cd in is more difficult to work with,.. ANyway you can move the head toward the outter area. (CD and DVD play from the inside out). Then you try again and see if the head is moving. If it is then you have an issue with the laser. You can clean it with a q-tip and alcohol. Then try it again. IF this wont work then it is time to see about a new one. NOTE that it can play CD's and not movies as there are two lasers in the unit.

With the power turned off, open the lid and then open the hatch where you put the DVD into. Locate the laser lens and notice that the laser lens is attached to an assembly that moves diagonally outward in a slot. First, slide this assembly outward. Second, slide it back inward. Now repeat, slide out then back in. Next, put in a music CD and let it play all the way through. (This re-calibrates the laser head assembly movement.) When the music CD is done playing, put in a DVD, you'll have a working unit again.
