Check speaker wires did not short out or the amplifier was not over driven. The usual cause to go into protect mode is a failed power stage.

How to hook up subwoofer for yamaha RX-496 2channels receiver

You need to connect the subwoofer to your receiver using the speaker wire connections on the rear of the receiver. Take the speaker input from the subwoofer to the A or B speaker terminals on the rear of the AV receiver. Then take the speaker outputs from the sub and connect to the main speaker terminals. Really you need to buy a receiver with a sub out to do it properly. Sell this one and buy another.

Yamaha S115V speakers making machine gun like sound when mic volume is turned up.

You need to check your specs on the amp and preamp. There is a mismatch with impedance or you are overdriving the amp input and internal feedback circuit is trying to clamp it and causes that sound. It is known as motorboating.

The audio outputs on the rear of the TV (if present) should be connected to the audio inputs on the rear of the receiver. From your description, it seems the present connection is incorrect.
