you need use the remote learning method to correct this, for some reason it seems that if you just push the buttons on the hdmi switch box ie, inp2 to switch to a second hdmi output, after about 5-10 min it will revert back, but if you use a infared remote set to switch it, it stays.
The axles may have substantial amount of in/out play, 1/4 inch or more. Does the axle in question actually drive the wheel or does it freewheel?
It's a typical differential so if you get both wheels off the ground and spin one wheel, the other wheel should rotate in the opposite direction.
The axles are retained by the differential assembly (inside transmission) so if the axle is truly falling out, to fix it will require transmission be removed and opened. Probably not an economical repair unless you can DYI or replace with good used unit.
