Do you have the phone on speaker or the earphone? There are differnet volume adjustments for speaker vs. in ear vs. the headphones. If one is really quiet, try one of the other ways to see if it is an issue with a speaker, or an issue with the phone?

Plug into itunes, do a backup, then do a system restore. This happened to me once, when I did this, the icon reappeared.

Your bottom speakers may be damaged and have to be replaced. Do you have the problem when listening through the top speaker with your ear?If not I can guarantee thats most likely your problem. You can take it into apple and get them to double check and confirm.

What is your current ios? Perhaps you need to upgrade?

Have you checked your settings and video settings for anything out of the norm? You can also try checking some youtube videos, they can be quite helpful

Sync the 3GS with itunes, ensuring you check all the options you want the sync profile to capture.
then right click on your handset in the left hand bar and select 'Backup'
when you get your i4, restore it to that backup and your data will be transferred.
