This means that the system wants you to reconnect the first player controller, usually in the 1 slot. It will do this if it thinks the controller is missing. I get this frequently on my xbox when the controller runs out of batteries.

Make sure the controller is plugged in all the way, charged and working properly!

Alternatively, you can power down the PlayStation and start it back up again.

Even though you console is able to read your other discs, the reason it isn't reading Guitar Hero 5 is a console problem. Some games read differently than others, the only thing I can suggest is to clean out the dust from inside the Xbox by spraying a can of compressed air into it.
