This is due to faulty DVD lens/head. Try cleaning it with DVD/CD hed cleaning CDs.If still the problem persists, contact near by service center for replacing the faulty Lens.

These types of dvd's are custom installs so the brand and model pf the player would have helped.
On them the sound is sent in 3 ways:
1 - usually there is a cable coming from its audio output connections to the AUX input of the radio. You will have to check the player to see if one was installed; if it has you must make sure the radio is set to use the AUX entry as a source and that the cable connecting them is intact. For testing purposes you should connect a separate amplifier to the player thru these output connectors to see if you get a sound; if you don't then the defect is in the player. Check the audio settings, if they are ok then the defect is hardware and the player will have to be taken to a repair shop.
2 - if the guy that installed it didn't wanted to bother with the hassle of removing the head liner to draw the audio wore then it has most likely just set the connection thru the FM modulator: the player has a modulator that transmits the sound thru the FM radio band; to listen to it all you need to do is to tune it at the frequency of the modulator - usually between 88 and 89.5 MHz.
3 - the headphones - very simple devices. The only thing you can check here are the settings from the player.
