You should look on the box or on your amp and find out how many watts it is. Once you have done that...any best buy or electronics store will sell amp wiring installation kits based on the wattage of your amp. When you purchase the one suited for the amount of wattage that you have...there are detailed directions included in the kit along with the wires needed to install what you need. According to your question...the only thing that you need going directly from you sub to you amp is speaker wires, and it should be quite obvious on the amp where the speaker wires go. If you only have on sub then you should have a mono output amp which only allows the use of one sub, if you have an amp that accepts two subs then the sub can be bridged to both outputs on the amp but you need to be sure the your sub can handle the output wattage from the amp or you can kiss ur sub goodbye. I would not recommend doing that so I cannot offer a solution for that but I can tell you that if you dont have a mono amp they are very affordable...but you need other wires to power the amp, and rca wires ran from the back of your head unit, also a ground wire. All which is included in the kit I mentioned above...just didnt know how much you had done already...if you have no wiring at all then make sure you purchase the right kit that inlcudes all the wiring above plus a fuse holder that is in line on the power wire...you may need to purchase some fuses if not included in the kit. It would be best to talk to a rep at the store you go to and they should be able to help you plenty just bring the info of your sub and amp...also if you not familiar with wire connectors be sure to ask if the connectors you need are included, and maybe ask about tools if you have not done this before.

A pair of JH4512-04's can be wired to 2 ohms if the subs themselves are paralleled. There's only one voice coil and it's 4 ohm. If wired in series, the final impedance to the amp would be 8 ohms. They cannot be wired to 1 ohm.
In bridged mode, your 4-channel amp is only stable down to 4 ohms. You can bridge 2 of the channels to provide a relatively low 200 watts RMS X 2. The best power solution for your subs would be to bridge channels 1 and 2 and wire one sub to it. Likewise, bridge channels 3 and 4 and connect the other sub. If you connect both subs in parallel, the impedance will be too low causing the amp to overheat and possible fail.
As for the settings: Low pass filter on, crossover somewhere between 80-100, input levels to match your receiver, and bass boost to your personal preference.

I would like to know how the plus and minus are
the plus is postive usually red and the minus is negative usually black. make sure you dont mix them up when you are connecting them to the acutal speaker or you will blow it and it would be useless then.

Take the cap out and just wire the amp to the battery (fused of
course!). Disconnect the speakers and see if it does it. It sounds like
a speaker wire could be shorting. Also checking power and ground is a
good idea.
What is the proper box
It all depends on the manufacturer's specs. For instance memphis makes a bunch of different 12" subs and each needs a different box. Also It depends on what kind of sound you want. If you want precise bass I would go with a sealed box. If you want loud then I would go with a ported box. Then the port depends on what the manufacturer recommends. what I would do is find their website or just a tech support phone number and get specs from them. If there is one place that can tell you a good box, it is the maker of the subwoofer. If you need help from there just let me know and I would be happy to help.

I used to have exactly the same subwoofer. It has dual voice coils which are driven independently.
Hope the diagrams help out. Just make sure you know how many ohms your amp is.

I think voice coil is
The voice coil can be properly checked with a voltmeter. Disconnect the speaker and use the OHM setting on the meter to check to see if it measures around 4 ohms. Is this speaker a subwoofer? Some subwoofers will be rated at 2 ohms. Either way, any speaker should have a sticker or stamp on it telling what the impedence is. Typically, most speakers will be either a 2ohm or 4ohm measurement. If the coil is bad, you will either get no reading or one that is much higher than the 2ohm or 4ohm. Hope this information is helpful.
Best way to wire the
Well, from what I can find, the best you could do is 2 ohm stable. This is only if you have a decent 2 ohm stable amplifier. In order to wire it to 2 ohms you will need to take the positives and hook them together and run one wire to the brigde positive of the amplifier. Then take the negatives and hook them together and run one wire to the bridge negative of the amplifier. Make sure you have a good enough amplifier to run this sub also. I would definately use a mono block amp that is 2 ohm stable and at least 1000 watts. I'm not sure if you have the 10",12", or 15" but 1000 should do it. There is one key that many people miss.......it is just as bad to underpower subs as it is to overpower them. And make sure you get a quality amplifier....if it is a brand new 1000 watt amp and it is under a hundred dollars you are not getting a good amplifier. Amps that I would recommend getting are gently used high end amplifiers, namely Memphis. I'm sorry I've rambled enough, but if you need anymore help just let me know. Good luck

If each speaker only has ONE negative and positive terminal, then its about 95% likely that they are 4 ohms.
If each has TWO sets, then you will have to m,easure it with a DMM.
Audiobahn car amplifier
Sounds like you have fried your output transistors. They can be replaced if u have the patience and know-how. Also check the resistors near the transistors. Some of them may need to b replaced too. I fu replace the transistors and the resisitors are still fried, u run the chance of burning up the new transistors again. I usually just replace both items since it's not that expensive and I'm doing the labor. What have you got 2 lose?

Sub repair
Well actually you can repair it your self with geat ease i may add,,
actuall all that can go in the speaker is the coil.. Yep the coil thats all
You will need a coil for it, a dust cap and the cone.
1.) using a knife, cut the cone off
2.) ulsolder the coil from the speaker
3.) resolder the new coil
4.) firmly fit and glue the coil to the new cone
5.) glue the cone to the metal edges of the speaker
6.) glue on the dust cap..
7.) have a beer your done..
You can get the parts at any local electronisc shop, I'd recommend you cut the cone and the coil off and take it with you when buying parts.
Repair, Replace
Fixing them isn't worth it just purchase some new 6x9's for less.
