On the underside of the handle there will be a set screw, usually an allen key that will need to be loosened. Once you do this the handle will pull right off. IS THIS A SHOWER ONLY? IF SO ,I WOULD REMOVE THE SHOWER HEAD FIRST THEN TURN YOUR WATER ON.THE HEAD JUST MIGHT NEED TO BE CLEANED AFTER 11 YEARS.

Two possible options i can think of. 1) the hot water valve at the hot water tank is partially closed. look for a lever that can be opened fully. the valve could have become faulty. 2) a copper line from the tank to the faucet might be bent somewhere in the middle

Pressure could be low at the source

Sounds like your cartridge is not properly seated and also may not be aligned properly. Remove it and apply a thin coating of plumbers grease to the cartridge and then work it back into it's position rotating back and forth to insure it is going down all the way.

Not easy but drill it out .

Change shower. if u want repair open the controller and replace rubber washel.
