I believe you can keep your number, but it depends on who you're porting to rather than porting from whether the port will work or not.

Also one page says:

Some service providers have applied for a waiver from number portability which is a request for a delay or exemption from providing the ability to port numbers. If your current service provider is one such company, then unfortunately Verizon Wireless will not be able to port your number at this time.

Finally, I would suggest report them to the fcc for not releasing the line if there's any problems. I would expect delays but threatening the porting dept with an fcc complaint will make you a priority port.

Well if you set the code then you will need to call customer care but if you never changed it then it is the last four digits to your cell phone...i just did that tonight and i finally figured it out

If you have never set up a lock code, your code should be the last 4 digits of your phone number, or at least that was the case for my phone through alltel. Hope this helps.

Depending on you network, you should be able to buy minutes, then load them onto the phone directly.
If you can't do that, you'll have to all the customer service number for your carrier and ask them to activate the phone again. (This might cause your number to change)
