Sound like you have a faulty fuel pump that is about to go out or possibly have a clogged screen. Either way, the bike is starving for fuel when you attempt to accellerate, but is just getting enough to start and idle.
Seaform will fix this trust me...go to autozone oickup a can put in 1/3 of can into approx two gallons off gas and ride at least fifteen miles hard and it will run alittle rough at first but it should kick in while you ride.
I'd check & lube all kickdown cables in the area of the throttle body, as well as follow any to transmission and lubricate well there too. It almost sounds like something may be seized and holding into lower gear.
on my cbr i had a problem w/ the cooling fan. so check to make sure it works. then your going to need to see if the radiator hoses are both warm, if not it needs a thermostat. the water pump is expensive and not likely to break. but, it is possible. my bike needed a thermoswitch, which turns the rad. fan on and off.
im pretty sure your transmission is going out on the car.you say it wont move in reverse and has hesitation going forward...id say start looking for another transmission or another car
