Hi.....
On your 91 Lexus ls 400.
If you remove the Rear passanger seat.
You will have axces to the fuel pump.
Remove the metal cover . Disconnect the wire harnes,
Disconnect the lines.
removing the nuts you will able to remove the fuel pump.

I have a 92 lexus and they had to take my whole backseat out and drop the gas tank to get to the fuel pump.The fuel pump is down by the gas tank

The fuel pump is inside gas tank behind the back seat unscrew the only top that's there and it is inside the gas tank. You will have to remove your back seat to gain access to fuel pump. remove rear seat then remove cover in the trunk, you will see a cover remove thatcover and remove 8 bolts holding fuelpump in tank,also dissconnect connector to fuelpump with bolts removed the pump pulls right out.hope this helps.
