Hello, and welcome to FixYa.
Please allow me to provide assistance to you.
The P-0401 code indicates an issue with the EGR valve. Normally, the most common problem is that the EGR valve sticks slightly, in the open position. To correct this condition, and to eliminate a potential for the engine to start stalling, when the valve finally sticks fully open, is to replace the EGR valve, the EGR transducer, and the EGR vacuum switching valve. This is a very common problem, and easy to rectify.
Thanks for choosing FixYa for assistance.
The graciousness of a FixYa rating for my input would be greatly appreciated.
Thanks in advance.

I am having a multiple cylinder misfire on my 2006 Toyota RAV4 with the 3.5L V6. Every cylinder is reporting a mifire. Each cylinder has it's own coil. I highly doubt that all 6 coils went bad at once. Does anyone have any idea what my be causing this?? Is that the problem you two are having?
