It sounds very much like the rear knuckle bushings. You can check them by having the car on the ground and pulling/push at the top of the tire. If they are bad, you will hear and feel how the rim seems to move independently of the car. The replacement is very simple and relatively inexpensive (about $120 for all 4 of them) if done by your self. The below video should make it a very simple process for you

Your car is swaying side-to-side because of a really serious suspension issue. It needs immediate attention. Like NOW. What can happen with no warning whatsoever is that you can instantly loose total control of the car. That translates into a potentially very serious accident. Such as going down the road at 70 miles and hour and suddenly flipping over a 6 times and then shooting across the median into a van full of children and everyone gets killed. Not the type thing you plan on.

If you are going to fix the car yourself. Using a good quality jack. First, to answer your question, jack up each rear wheel and remove the tire and examine the suspension system parts. You will find some sort of component that is very loose. Such as a bushing that has no rubber left in it, or a ball joint that is about to let go. If it is a bushing, then it is not as serious. If it is a worn ball joint, it may not be so easily detected. However a worn ball joint is deadly serious.
If you are not a skilled fix it yourself type of person, then the next trip or tow should be to a honest certified mechanic. I'm not exaggerating when I describe what can happen. A good friend of mine in a relatively new SUV flipped over the edge of a bridge with his young wife in the car. Luckily they lived. There wasn't a hint of warning.
