I have heard somewhere that this is caused by a faulty vacuum check valve which is black and white plastic and is located near centre above the rear of the engine up against the firewall. vacuum drops when you accelerate so there is not enough vacuum to keep various vacuum operated heating/cooling system valves in their selected position . the check valve keeps sufficient vacuum in a reservoir tank when vacuum at the engine manifold drops.

yes, volvo,s have a problem with this, remove clean and reinstall the pcv filter or replace it,excessive crank case pressure can allso lead to blowing engine seals and gaskets.

First check for holes or disconnected air inlet hoses, that will give the sensor/meter a problem so it cannot send the correct signal.

Very smokey (smelly) exhaust

Grey-ish smoke sounds like rich mixture, not oil burning.Grey-to black is usually excess fuel.Whitish smoke is coolant being burned-bad news! If rich mixture, check for air intake leaks (air intake hoses/manifiold gasket),MAF,fuel pressure regulator.
