I had the same problem all you have to do is call 1888mitsu2008 you'll need your vin # as well as the radio serial and part # it was free they walked me through the process all it took was about 10 mintutes of my time
Check in your owner's manual packet. In my Audi, there was a
supplemental guidebook for the audio system, and the security code was
on a card in that packet. If it's not in there, you can go to the
dealership with the car to get it - they can look it up based on the
VIN of the vehicle.
You'll need the title to the vehicle (to
prove it's your car), photo ID (to match you to the name on the title),
and the car itself. The dealer will then put the code in for you (make
sure they give you the code written down, and show you how to enter it
for the next time you need it). The downside to this is that they'll
probably charge you labor for doing this.

You will need to pull the radio and record the serial number and model number off the radio chassis, take that info and your proof of ownership for the car and the dealer should be able to give you that info for free by using the manufacturer online info service they have.

I had the same problem after replacing the battery on my Montero. I bought mine used but luckly the pervious owner had written the 4 digit number down in the owners manual. After finding the number, I had to enter it into the radio by using the station preset buttons. If you can't find or don't have it in the owners manual. You will have to get the model and serial numbers off the radio and contact Mitsubishi or another source to obtain the correct code to reset the radio.

I have just successfully re-activated my radio.1. Call 800-222-0037
2. give them your personal info
ie. Name, address, phone # etc.
3. Provide the VIN #
3. Provide the radio code
( I just pushed the plastic piece aside to
reveil the code on the top left corner of the
radio.) yours may vary.
4. The customer service rep will give you the code
5. If your radio displays off, leave the key on for
1 hour and then it will shut off. Turn on the
radio and enter the code.
