SOURCE: 1991 Chevy Silverado Rolls while in Park
Hi, This looks like the Pawl in the gearbox has been broken/damaged or even left out when the box was overhauled.
It lock the gearbox whilst in park so acting like a brake.
It could have occurred if someone put the vehicle in park whilst still moving.
Hope this helps?

SOURCE: What transmission to look for in my 1991 chevy silverado
The most common transmission for you vehicle is a 4l60e, Some come with a 4t65e. The difference is the gear settings. The housing is identical.
I would get the information off the transmission.
There is a I.D. plate on the side of the transmission that will tell you what type , I would clean off bothside of the transmission with a rag and basic cleaner and look for the infomration.
Good Luck

SOURCE: I need to know where the vacuum hose from the
Behind the carb on the intake there should be a small vacuum nipple. That is what it is for. If you can't find one there or it is being used, you can splice into it or any of the other vacum lines with a T, except the distributor. Hope this helps.
