I have a 2005 Avalon that when first started then put into reverse, there is a large grinding noise coming from somewhere around the back speaker. Lasts around 10-15 seconds.

Have the constant velocity joints looked at. Make sure there is enough grease in them and the rubber boots haven't been torn. Also have the engine mounts looked at. Worn rubber parts in the mounts can make that knocking sound when you go into reverse.
the ting with brake pads is that they seldom wear square
looking at the outside edge may be 40% but inside near the center , metal on metal

It's your exhaust mounts or broken exhaust bolts. It does it in drive st idle n under load because the motor is sitting in rubber mounts made to dampen vibration. Therefore the motor tilts to the left in reverse pulling the exhaust away from your under carraige, and to the right in drive causing the loose exhaust to vibrate against something. Check your heat shields also. They fall apart n do the same noise. Piece of cake.
If it is an automatic transmission, first check the fluid level. If it is low, fill it up and try it again. If the level is fine, then there is a bigger issue and you should seek a professional's help.
