When driving down road with heater fan on high the fan will slow down (if not stop all together) when she hits a bump. Have checked connection to fan and taken out fan to see if they were loose. This is often caused by a lose cable to your blower motor or your blower could be going bad. You can this by removing the cover underneath the glovebox. This will give you access to the blower and harness.
Bump the heater with your hand to see if the motor will try and run,or see if there is voltage to the blower motor,if there is,then the blower motor has gone bad.
2 things come to mind: can you hear the fan turning?
if not check for power at fan location. If there is power and the fan isnt spinning replace the fan. If there is no power there is a problem with the fan circuit which includes wiring fuse (you stated were ok) and Fan speed switch (might be a module)
Or there is a blockage it is possible for your heater core and evaporator to plug up with dust .
