First look at belt if loose worn cracked glazed or just plain old and worn out change because a bad belt makes noise when slipping on pulleys ------if belt is good than look at belt tensioner ---- and pulleys to all components ---and listen to the area when vehicle running for grinding or squeal

Not too bad. Not much room, just enough to drop the starter. It is probably easier to go up through exhaust if you can get your exhaust shield off the manifold .

Idle on 1996 lexus ls400 goes real high up and down

More often than not this is caused by a large vacuum leak on the engine, the surge up and down is the computer trying to slow the idle. Use a bottle of propane gas and a torch adapter with a rubber hose on it, spray the gas around the top of the top of the engine and alos around any vacuum hoses from the engine, if you hit a vacuum leak the engine will sound much different, repair as needed.

My 93 Lexus LS400 wont make a sound at start

Since work was recently done to the vehicle I would look to see if any connections were left unplugged or inadvertantly knocked loose. Make sure to check the battery terminals and cables to ensure tightness and a solid connection.
