Clean the throttle body bore and plate and check the Idle Air Control passageway and the tip of the IAC .
With the PCV unhooked it will idle high because you are creating a vacuum leak and allowing unmetered air into the engine. Doubtful the PCV valve is the source of your problem. You may have an issue with the idle air control valve or another sensor. Have the codes scanned to see if there is anything that may point you in the right direction. Driving with the hose off may compromise the operation of the power brake booster to lack of vacuum

It could be a lot of things, check the gas cap, vacuum leak, IAC, may need a tune-up ( check plugs, wires, air filter), seizing pulley, timing issue (check CKP sensor and CMP sensor), sticking valves, misfire, Fuel line pressure, or an issue with CAT or muffler.Good luck.

You have the wrong type hose on the pcv valvle. It isn't vacuum hose.

If it is the same engine design as the F-150 ( think the '98 was still the 2V V-8 ) it could be the IAC ( Idle Air Control ) is gummed up.
The F-150 it is mounted on the back of the Throttle Body ( have to take off the plastic throttle body cover to see it ), held on with 2 bolts with 8mm heads ( could be 9mm ??).
Worth taking a look, also take a look at the VAC line to the back of the throttle body, and check that the IAC connector did not get unplugged or the wires damaged from the other vac line change

Here are the most common causes of idle surge, stalls at stops, slow idle speed, erratic idle speed, rough idle and engine hesitation (and other problems), it is in most cases the idle speed control air-bypass valve and or throttle valve and upper intake, these area's get full of gunk and combustion residue over the miles and cause idle issues (stalls, low idle) like yours, Get a can of intake cleaner from any local parts store, not carb spray, intake cleaner, it is made by a company called CRC, remove the air intake hose to the engine, hold the idle high so the engine won't stall, then spray the can of cleaner into the intake while keeping the engine running, use at least 1/2 the can, shut down the engine and disconnect the battery for 5 minutes, then restart and complete a number of mixed driving cycles, town, freeway, stop and go etc., after a few days the problem will go away as the system will relearn to the clean intake
