You may have a bad air mass sensor (air flow meter) Either that or a very restricted exhaust, like a catalytic converter plugged up or came apart and is restricting the exhauxt flow. A restricted fuel filter, if you haven't changed one in a very long time, could also cause your symptom.

You'll probably have to change the clutch plate on the flywheel. You'll have to get a floor jack, but a transmission jack is easier to remove the transmission. To remove the transmission you'll have to remove the tortion bars on both front wheels. Also, you'll have to remove the crossmember aft of the transfer case. You'll also have to remove the front and rear drive shafts. The manual says you should keep the transfer case from leaking oil out the back. but, you can just top it off after you're done. Be careful not to damage the seal seal for the rear drive shaft. Anyway, once you get the transmission and transfer case out (you'll have to remove the shift and transfer case levers), you should have enough space to remove the clutch plate from the flywheel on the engine. You should have an alignment tool to make sure the clutch plate is aligned with the crankshaft so you can put the transmission back on.

You need your idle speed adjusted, or you have bad gas, or you have dirty fuel injectors. I would put some injector cleaner in the fuel tank. Go fill the car up. Then if it doesn't improve by the end of that tankfull, I'd take it in for a tune-up.

What did you use to clean the MAF? Most cleaners for throttle bodies and carburetors will damage the MAF. You must use cleaner specifically designed for MAFs to ensure that they will not be damaged during cleaning.

Change your plugs and run a couple of bottles of injector cleaner through it. Sounds like you have some carbon buildup. Also check the catalytic converter for excessive back pressure. i hope this helps.

Have you checked fuel pressure at idle and 3k TO 4K RPMS? I once took a crown vic to a local dealer, wanted $1500 for new cats. I did a fuel pressure test, 15 lbs at idle, 0-5 at 3K RPMS and went to 0 lbs. when punched accelerator. Was a $6 fuel filter.
