If any fault codes are stored the o/d light starts flashing. Most Hydraulic problems will not set any codes. But a line pressure test will show what is going on. I know that the RWD transmission designated AOD FIOD AODE A70W had problems with a sun gear shell weld that would cause no 4th gear (mechanical ) failure this could be possible in your axod, axode transmission. as well.

Check transmission fluid warm idling wipe stick and check several times fluid seems to stay in tube and gives you false readings there is a retainer plate in valve body that breaks and causes inadvertent downshifting this requires pressure gauges on transmission to verify problem.

My T45 transmission is stuck in first gear and bogs down when I shift it to reverse

It appears this is a 5 speed manual transmission. There may be shift linkage problems between the shift lever and transmission. also bushings. I strongly recommend a repair manual specificly for this transmission. possibly filespeep.com or manualslib.com

Check the fuel pump output. Some will work but not have enough output for the instances you describe. Find a place that can test the fuel pump pressure.

follow speedo cable to the transmission ,it should screw on at that point. next take out 1 bolt for y shaped hold down fork and check for a stripped speedometer gear , its plastic pull it straight up and out if teeth are missing check the gear in transmission connected to that one. also since its apart try turning the cable while someone watches it, if it turns smoothly then its ok just replace transmission gear.and reinstall cable. if it makes the speedo jump or turns roughly replace cable as well.your local auto parts stores should have these, if not I know the dealers will.

1997 Ford Windstar

It could be the vacuum modulator on your auto transmission, sometimes when vacuum hoses fail, split or whatever, they can suck up impurities causing other downstream items to fail. Also check mechanical linkage form shifter in cabin to transmission gear selector arm. If jumps were happening these might have worked loose. Best of luck!
