Then you'll have to remove the inside door skin. It's not too difficult. The hardest part is if you have manual rollup windows, you need to remove a clip behind the window crank and sometimes it will fly off and get lost. Remove all screws you can find (some are behind plastic covers, like for the inside door opening handle, after removing that one slide it to the front of the car to remove the handle), one or more may be hidden under the bottom carpeted area if you have that (run your fingers around the edge area to feel the holes). After all that the door skin should be held on by 'panel pops' sometimes these break if the cars old or if your not carefull. You can get replacements cheaply enough.

I have the same problem on a Toyota Tercel 1997. I am trying to remove the door panel with the door closed (I can not open the door because the door knob is stuck in the lock position). I think I will have to cut through the panel. Have you found something ? My e-mail is [email protected]

It should be able to be replaced without removing the door panel.
However, removing the door panel is a very simple process. Remove the bolts through the armrest, and pop the door panel loose (it's held on with plastic friction clips) They do make a tool for removing the panel, (just a couple of bucks) or if you're carefule, you can do it with a putty knife and a screw driver ((basically you pry the panel away from the door, replacement is just popping it back on.
Once it's loose, slide up, it is hooked over into the window channel (you do want to have the window down).

On the inside trim, you can remove screws that are by the door handle, arm rest, under door and side of door and then gently pull back on the panel as there will also be retaining clips attached and you don't want to break them. You'll need to lift the door panel upwards in order to get it off the window ledge. Pull back the weather seal and you'll be able to get to remove the bolts securing the outer handle on
