The code is telling you where the problem is. Try replacing the HO2S sensors as the cpu is having trouble keeping the air /fuel mixture within limits. Check the cat converter for cracks or blockages . Have a pressure test done on the fuel manifold to check for pump/filter and pressure regulator problems. Check the ambient air temp sensor and coolant temp sensor as they also affect the air/fuel mixture.

You should definitely replace the fuel filter though this probably won't fix your hesitation. It wouldn't hurt to replace the spark plugs this is usually the culprit on these engine. I have the same engine in a '99 caravan and found the for some reason the spark plugs on cylinder 3 and 4 loosen up after one year and cause a misfire between 45 and 50 mph under load (when mildly accelerating).

You may have to replace the fuel filter, the fuel pressure regulator or the fuel pump.
First I would check the fuel pressure and also check for leaks. If there are no leaks and the pressure is fine than your fuel injectors are probably clogged or you may have an ignition problem or even a compression problem.
If you don't have good pressure and there are no leaks I would first replace the fuel filter or the fuel pressure regulator. If problem still exists I would then check and replace the fuel pump.

the most propable cause is you have a vacume leak. Check all intake hoses for leaks and spray brake cleaner around intake gaskets if rpms change then you found a leak also clean your mass airflow sensor
