clean throttle body,and check for vac leak

Have you tried disconnecting the EGR valve?

try to reset your valves to specs... I find in many Honda's I work on that people think they can adjust there valves because they make a tapping noise.... If your car ran fine then this happened you can check under the throttle body directly under neath there will be two 10 mm bolt on the bottom inside there will be a plastic screw make sure it is all the way in sometimes they loosen up..... when that happens runs fine cold then the erratic idle begins when warmed up..... hope I was able to help.... :)

some older motors will have worn vacum control valves in your emssions control system a leaking valve or broken vacum hose would do it.

Sounds like your idle air control valve is bad, causing the hard start and erratic idle, up and down.

The accelerator cable need to be adjusted to higher the revs a bit.

Check for split hose's around the inlet manifold.Sounds like a vacuum leak.

take the dash out and make sure all the grounds are in good condition and also the grounds around the distributor
