This problem
is commonly caused by a dirty automatic idle speed control valve and
throttle valve but always run a OBD2 (1996 and newer) or a OBD1 (1995
and older) fault code as well as the cleaning procedure. Buy a can of
throttle valve cleaner (do not use carburetor spray cleaner!) from
NAPA or Carquest (made by CRC chemicals) and spray it into the air
intake while the engine is running, use up about 1/2 the can, engine
will try to stall hold the speed up, shut it down and let it soak for
30 minutes, restart and blow out the remaining fluid, shut it down
and disconnect the negative battery cable for 5 Min's to reset the
base idle control
SOURCE: engine stalls when put into gear, idle fines in park
This is a comon problem with GM cars. Most likely cause is the torqe converter clutch lock up solenoid.

the problem you are discribing would lead me to belive your fuel filter is clogged which is a simple fix 3 dollar part at any part store or on the worse hand your fuel pump itself could be weak you can open the gas tank cover and have another person turn the truck key to the on position without going far enough to start you will hear a faint whining noise if it is going bad or a louder noise if it is fine Good Luck
SOURCE: My 95' Chevy diesel k2500 stalls out when im Driving
I have replaced many injector driver modules on these diesels. GM has redesigned it and i have never had a replacement one fail. This is a known problem for the 6.5 diesel. Unfortunately its about a 500 dollar part from the dealer, and requires intake removal, and some very tight work space to replace.

SOURCE: 1999 chevy silverado service 4 wheel drive light
I had a similar problem with
the Service 4 Wheel Drive light which came on intermittently then became more
consistent in the message center display. It would go into 4 wheel drive if the
message center did not display the Service 4 Wheel Drive message in the message
center, but not when the message was displayed. I found the transfer case
switch module in the dash was defective, and it has been working with no other
problems for three years.
Here are the common cause of surges, stalls at stops, slow idle speed, erratic idle, it is in most cases the idle speed control air-bypass valve and/or throttle valve and upper intake, these area's get full of gunk and combustion residue over the miles and cause idle issues (stalls, low idle) like yours, Get a can of intake cleaner from any local parts store, not carb spray, intake cleaner, it is made by a company called CRC, remove the air intake hose to the engine, hold the idle high so the engine won't stall, then spray the can of cleaner into the intake while keeping the engine running, use at least 1/2 the can, shut down the engine and disconnect the battery for 5 minutes, then restart and complete a number of mixed driving
