it needs a new fan motor not real hard to do. remove fans from radiator and change motor or take to shop will cost around 300.00 to have fixed at shop around a 100.00 if you do.
It might be a loose connection right there where the fuse box is by the battery. Is it a maxi-fuse? Pull the fuse box out enough to check underneath where the wires hook in. A loose connection would exhibit burning, pitting, melted insulation or plastic, or other evidence of high heat generated by a loose electrical connection. Another place would be the junction block where your wiring harness enters the car interior, usually under the driver's dash. If the coolant fan is on that circuit you could have it checked Or just hot-wire it yourself and see if it runs quiet and normal and seems in order. Just unplug the fan connector and use two jumper wires from the battery to the fan motor terminals. If the motor is shorted out, it will either be dead or very noisy trying to spin.
Bearings in the blower motor are shot. Only fix is a new blower motor. If you have no experience in auto repair, I would take this to a qualified shop, as most of the time you will need to disassemble the dash to get to it.
I have seen the blower motors mounted two ways. In the engine compartment, on the fire wall, passenger side. And under the dash passenger side, have to remove glovebox to gain access.
first remove the ashtray. there will be 2 screws where the ashtray used to be. remove those. then you just have to pry the panel off it will feel like it will break but then you hear a pop a couple of them. and then you replace the lights
