The temperature of the air coming out of the ducts is controlled by a door that moves between the heater core and the AC core, it is called the "blend" door for obvious reasons as it blends the hot and cold air streams to give the desired air temperature at the dash vents. The door is controlled by a small gear drive DC motor called the blend door actuator, what happens in most case is the actuator is damaged by a blend door that is warped from heat being hard to move or not moving at all which is the case with your truck. The blend door and its actuator should be replaced as one repair, the dash must be removed from the truck to access the air ducting that contains the blend door, you absolutely need a service manual to complete this job, it takes about 10 hours.

First try rotating the tires to the rear. Does it go away? If you apply the brakes does it pull to the left? Could be a draging caliper or the slides may be hanging up.
SOURCE: wheel bearings on 2005 HD 2500
I just came across your posting, If you're still having this issue, it may be due to the offset of your wheels. Too much offset can cause excessive load on the outboard bearings. If you have wheels that move the centerline of the wheel too far outboard of the bearing, then you will generally be putting too much stress on the bearing/hub assembly. as you know your vehicle has a unitized bearing assembly. If the bearing is being overloaded on the outer edge by too much offset it will fail over time. hope this helps

Thank you very much. After driving today for 4 hours and the only way I would get heat is to move the heater controls up and down fast and then leave it in the max heat position and I would have heat for maybe 30 minutes and then it would go cold again, repeat the step and I have heat again. So I think your solution it right on. Thank you.
all controls run at medium speed except fast runs fast will ot turn off
