The radiator cooling fan control module (also known as "radiator cooling fan relay", athough is is a solid-state control module - not a relay) is located on the left frame rail just behind the front bumper/headlamp area. You must to get under the vehicle and look up to see it. It looks like the picture below.

The age of your vehicle also guarantee's a vacuum leak. It may sound silly but the truth is rubber grommets, and hose's shrink and dry out over time and the harsh environment under the hood makes it worse. Check all vacuum lines and fittings attached to the intake manifold, valve covers (pcv valve), and be sure to check for a vacuum fitting on top of the intake just behind the throttle body. Does it fit tight in the hole? You can seal loose fittings with a high-temp RTV (Red) silicone sealant/adhesive. Several small leaks add up and your engine, heat/AC, and transmission will be all to glad to tell you about it.
Engine fuse keeps blowing causing van to shut off
For the engine fuse to blow the the nearest ground that affects the engine fuse is the heater circuit of the 02(oxygen sensor) the harness has rub through you will have to open the the harness and inspect

one of two things, belt is old and stretched out and slipping over pulleys, or you have a bad bearing in your alternator or other accessory. or one of you accessories (such as a power steer pump) is starting to go out and having a hard time turning causing your belt to slip
