lots of questions could be ask here.. how about the pickup coil inside the distributor or the module inside the distributor?? have they been changed?

some do require shims but most of the time u can get by with out them depending on where the starter gear hits the flex plate

There is also a fuel pressure regulator you might want to check that out. Other than that you could have a bad line some where.

Sounds like a cracked cylinder head...

I would start with the air and fuel filters. Next would be fuel pressure and spark plug wires. Then I would check the ma's air flow sensor. Not enough informationinformation, with a limited problem I can only give a limited attempt for solution.

have a compression test done as it appears that you have a gasket problem or a cracked head.

If it's your heater core, unless you're a mechanic you probably should get professional help. The heater core is located on the firewall and many parts need to be replaced to get to it. Do you see water on the floor inside the vehicle, Many times a heater core will leak inside.

two things pop to mind with this truck, first these motors are notorious for having bad intake gaskets take a flashlight and look very carefully around the valley of your intake/block-head seal you'll possibly see either antifreeze leaking or a weird looking whitish or greenish sort of foam you can also use propane around the seal and look for obvious idle changes, I have done tons of these intakes in my shop, the other two things I would suspect if that looks fine are 02 sensors, or maf sensor.
