Why does it vibrate between 65 and 75 mph?

My car takes forever to start when left overnight and when it does start there is loads of smoke comes out the back, what do I need to do to resolve this?
check the heater plugs but if this is a diesel then often the cause is the fuel filter which is at the top right hand side of the engine ,what often happens is the pipes or the fuel filter head itself are drawing in air .This is a plastic housing and pipes and often they break or distort also the screws that hold the filter head on which screw into plastic dont hold the head on tight enough and they suck in air .Failing which it could be the EGR valve sticking if so pull the small plastic vacumn pipe off EGR valve but some newer ones have a electric operating mechanism so pull the plug out and see what happens .It will perfectly ok without the EGR connected and still pass the annual inspection without anyone knowing
Fuel filter
Most newer vehicles have the fuel filter mounted on the inside of the frame rail, driver side, usually back of the driver seat or near the driver seat. Jack up and support the vehicle. Go underneath, and look for the metal tubes running along the inside of the frame rail. If the tubes go into the passenger compartment, your filter may be under the backseat, driver side - remove back seat and look for small door on the driver side. If under car, remove mounting bolt (s) first. Then wrap your wrists with rags to keep the fuel from running into your armpit ( man, that BURNS!!). Wear safety glasses, as a fuel injected motor has much higher pressure than a carburated motor and the spray can blind you. If under the seat, have plenty of rags handy to mop up the spilt fuel and protect you eyes as you release the pressure from the fuel tubes.

The first thing I would do would be check the fluid level in the radiator. Sounds like the car is overheating under load due to lack of coolant. If the coolant level is ok then the radiator cap may be leaking pressure and causing the car to overheat.

Not selecting 1st gear when stopping at lights
would need a drive to be honest to know whats wrong ,Now in my time cars had no syncro on first or second gear so you had to stop and be stationary to engage first gear which i still do if the cars moving abiet very slowly then i use second but a lot of modern cars like this have out of square engines unlike a old humber or morris with a long stroke engine and a starting handle for a flat battery morning.Now is this not going into gear when moving and ok when stationary or all the time ?

The first thing you must do is have the AC system pressures checked, you may have no refrigerant in the system due to a leak.
