You have to use one or the other. If you use the earpiece or headphones, you have to use one with a microphone in it. If you use the speaker in the phone, you have to choose it from settings during the call and use the phone's microphone and speaker.

According to the user guide which you can read at the link below, the factory-set USER lock code is 1234. The document also covers lost codes, saying, "If You Forget a Code, PIN, or Password Note: Your phone's 4-digit unlock code is originally set to 1234, and the 6-digit security code is originally set to 000000. Many service providers reset the unlock code to the last 4 digits of your phone number before you receive your phone. If you forget your unlock code, try entering 1234 or the last 4 digits of your phone number. If that does not work, do the following (and shows a graphic of doing...) Press the OK key, then the keypad keys to enter your security code (or try the defaults I mentioned above) and press SUBMIT to enter the code. Here is the link to the page I was quoting from.
